The purpose of this assessment is to broaden each student’s understanding of the module
content using a case study and assessment tools
Case Study: Brian is an 84-year-old retired farmer in a rural area in Northern Territory. He
was recently assessed following a minor motor vehicle accident and diagnosed with vascular
dementia (Montreal Cognitive Assessment score of 15 out of 30). Other medical problems
include type 2 diabetes for more than 15 years, hypertension, coronary artery disease
(coronary artery bypass graft 12 years ago), stable chronic heart failure, abdominal aortic
aneurysm, chronic kidney disease (estimated glomerular filtration rate of 34 mL/min), benign
prostatic hypertrophy, cataracts (visual acuity of 20/80), and chronic back pain. He is currently
taking eleven medications every day.
Brian lives with his wife, Betty, in their own home, which they have lived in all their life with no
modifications. Nonetheless, Brian can perform all basic activities of daily living. His appetite
is adequate, but his wife and son note that in the past 6 months he forgets to eat regularly if
not prompted. Brian has also been taking a few tumbles mainly outside home on uneven
surfaces but had several near falls also inside the house. Betty has taken on the responsibility
to look after finances and grocery shopping. She is feeling overwhelmed since she is not a
driver.

- Discuss whether frailty is an issue.
- Prioritize care needs and strategies to maintain Brian’s independence. Provide your
rationale with the support of scientific evidence. - Compare and contrast strategies at individual, system, government, and global levels to
address the identified concerns/issues, based with scientific literature and health
policies. Discuss the impact of the issues on the current ageing demographic. - Finally, students are then required to produce one long term and one short term goal for
this case, summarising strategies which includes interventions and outcomes to support
them in terms of healthy ageing. Include evidence and theories of healthy ageing where
appropriate.
Formatting: Prepare your paper according to APA 7th edition format. This includes the title
page, headings/sub-headings, and references to be formatted according to APA style,
however you do not need to write an abstract. Use your name as the running head. Format
papers double spaced, 12 pt. font. You may be able to draw on material you used in weekly
discussions and course content. - Weighting: 40%
