The interview with a nurse manager revealed challenges in coordinating care transitions due to poor communication between departments. Implementing Kotter’s change theory could guide the process by establishing a sense of urgency and building a guiding coalition. Leadership strategies like transformational leadership would empower team members to embrace interdisciplinary collaboration. Evidence from recent studies supports this approach as it enhances patient safety and efficiency. Daily huddles structured around SBAR communication have shown to reduce misunderstandings in similar settings (Müller et al., 2018, https://doi.org/10.1186/s12913-018-2998-3). This method ensures all voices are heard and aligns actions toward common goals.
