Part III:  Population Health Prevention Strategies (Unit 5)

Component: Community Interviews

Directions:

Interview two Community Members (for example: police or fireman; school teacher, community member [no official title], mechanic, or librarian; business owner or store clerk/manager) AND one health care team member (for example: public health nurse, school nurse, Red Cross nurse) to determine their perspective on:

  • Perceived health/illness beliefs, values, attitudes and practices that influence health
  • Community Strengths
  • Community Needs
  • Level of Safety
  • General Concerns

This portion of the assessment will give you an idea of how people in the community see the community they live in. Please note, it is important to select a healthcare team member with a role that allows insight into the needs of the community, as this person will be integral in determining the priority health need of the community which is needed for the collaborative component of this assignment.

Use the provided template to document the interview components, using complete sentences with accurate grammar. Submit the template to the Dropbox along with the presentation described below that will detail the priority health needs for your community.

Component: Collaboration with Community Healthcare Team Member

Directions:

In the final portion of the community assessment project, you will create a presentation that details your analysis and plan for the community. Using the data identified through the windshield assessment, the data analysis, and the community interviews, work with the healthcare team member (the same person you interviewed) to determine the priority health need within the identified community and develop an appropriate plan of care. In collaboration with your community healthcare member develop a community diagnosis and an intervention plan that considers assessment findings with a focus on determinants of health, available resources, and the range of activities that contribute to health and the prevention of illness, injury, disability, or premature death.

  2. Presentation slide requirements: Create a slide presentation including the following slides with bulleted content and graphics as applicable:
    1. Title slide with an interesting title, your name and the date.
    2. Analysis of the collaboration with the healthcare team member
    3. Develop a community (not individual patient) diagnosis. Use the following format: Health Risk/problem/potential of/for (specific injury, illness, or potential of, complications of, etc.) among (specific population) related to (supporting evidence of risk factors/DATA) as evidenced by (contributing factors)
    4. Possible solutions based on evidence that can address the targeted need (3 Scholarly sources required)
    5. Two SMART goals (Specific, Measurable, Achievable, Relevant, Time-based)
    6. Two specific intervention strategies (Remember, this is at the community level…not patient level)
    7. Two partners or community stakeholders needed for collaboration
    8. Reference slide (Minimum of 3 sources required)
  3. Presentation Design and Formatting:
    1. The presentation should be organized, attractive, creative, and professional.
    2. Use contrasting uniform font and background colors and appropriate font sizes to enhance readability. (Review the Microsoft resource for Making PowerPoints Accessible.)
    3. Use short, bulleted points and limit the use of large number of texts.
    4. Use smart art, graphics, and quality images when appropriate to support or explain the content and reinforce the presentation.
  4. Presentation Delivery:
    1. Student presence on camera is required while presenting on screen. Presentations that do not include student presence on the screen will be scored as a 0 grade.
    2. Access to a portable or built-in computer camera and microphone for this recording is required.
    3. Adequate room lighting and recorded video and audio quality are required.
    4. Do not read from notes or slides during the presentation.
    5. Prepare by completing each slide beforehand, write notes to use as speaking points or a script, study the material, and rehearse for time management before recording.
    6. Present the content in a professional manner using conversational tone.
    7. Be mindful of personal attire, background, language, and possible distractions from family, phones, and pets.
    8. Limit presentation to no more than 5 minutes in length.
  5. Presentation Technical and Submission Requirements:
    1. The use of ScreenPal is highly suggested. You are required to be on-camera.
    2. Be sure to use the ScreenPal setting which captures both you and your screen for the duration of your recording. Remember, student presence on camera is REQUIRED.
